Here are two different tools you can use when carrying out a Cancer Care Review.

Tool number 1:

The Holistic Needs Assessment (HNA) and associated care plan is a simple way of finding out what really matters to the person with cancer. 

An HNA ensures that people’s physical, practical, emotional, spiritual and social needs are met in a timely and appropriate way, and that resources are targeted to those who need them most.

An HNA is a simple questionnaire that is completed by a person living with cancer. It allows them to highlight the most important issues to them at that time, to inform the development of a care and support plan with their nurse or key worker. The questionnaire can be completed on paper, or electronically (eHNA).

Tool number 2:

National standardised Cancer Care Review templates have been developed by Macmillan and made available in EMIS Web, TPP SystmOne and INPS Vision. These care plans are not always patient-led, but the information will be read-coded. Click here to find out more.
