The Learning Disability Programme at Improvement Cymru has put together materials to support our stakeholders to spread the word about Health Profiles.
On this page, you can find a range of bilingual resources that we encourage you to use on your channels to communicate with your own audiences.
Linking to the Health Profile
The main Health Profile webpages are hosted on Improvement Cymru’s part of the NHS Wales Executive website. These pages host the Health Profile templates, guidance notes, and other useful information.

Materials for Local Authorities, private sector care providers, and third sector organisations
The purpose of these materials is for you to encourage people with a learning disability, their families and friends, carers and support staff to complete a Health Profile. You can scroll sideways on the rows below for more assets.
Materials for NHS Wales Services, Trusts, and Health Boards
The purpose of these materials is for you to raise awareness of Health Profiles with staff who work in healthcare services. They should ask patients with a learning disability for a Health Profile or may be presented with one.
