Practice nurses, GPs and other primary care clinicians who want to start carrying out Cancer Care Reviews in their GP practices will find this information useful. 

A Cancer Care Review (CCR) is a holistic conversation between a patient and their practice nurse or GP about their cancer journey.

A CCR can help patients to:

  • talk about their cancer experience and concerns
  • understand what support is available in their community
  • receive the information they need to begin supported self-management. 

Many people living with cancer describe the time when they leave the acute setting, and go back into the community as feeling as though they have fallen off the edge of a cliff (Macmillan Patient Experience Survey 2016).
